What to Expect From Your First Visit

Your initial appointment is a consultation visit. During this visit you will undergo an exam and a complete medical history will be taken. The surgeon will discuss your individual diagnosis and treatment options. You will then receive an individualized treatment plan, with all associated fees. At that time, you may also schedule the surgical or follow-up appointment.

Please assist us by providing the following information at the time of your consultation:

  • Prior to your appointment, please complete the new patient registration forms on this website. This will expedite your check-in process. If you are unable to complete the online registration forms, please plan to arrive twenty (20) minutes early to complete the forms.
  • Your surgical referral slip is generally provided by your general dentist or physician.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, it is imperative that you bring both cards with you to your appointment.
  • A form of ID.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a natural parent or legal guardian.

A preoperative consultation, history, and physical examination are mandatory for all patients.

New To Our Office?

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.). It is very important to accurately complete the health history forms. Answer each question “yes” or “no”.

X-Rays

We take an x-ray on each new patient, regardless of any x-rays that may have been taken at another provider’s office. We do this to ensure our surgeons have an excellent quality x-ray film that allows them to diagnose and treat you. We will file with your insurance company for reimbursement based upon your individual plan benefits.
